Insurance coverage can differ based on individual plans. To ensure that Dr. Pham is in-network with your insurance, please contact your insurance company.
We are not currently in-network with Kaiser, Tricare, or Medicaid.
We can provide a superbill for out-of-network reimbursement. Please call the office for more information, including details on out-of-pocket costs.
Concierge membership payments are not billable to insurance but may be covered by an HSA or FSA card.

Dr. Singh is not a part of Dr. Pham’s concierge practice, there are no associated membership payments.
